After discharge from the hospital, our staff may need to reach you at home to coordinate your care. They may need to review laboratory tests or adjust medications. These phone calls are part of our Telephone Care Program.
What to do if you have questions after discharge?
- If you are a Primary Care Patient:
Call directly to your primary care team if you have questions about your treatment plan, test results, medications, etc. You can locate the phone number of your team on the card provided to you by your team clerk.
- Telephone Triage:
Telephone triage is available if you begin to experience new medical symptoms that are not of an emergency by nature and which you require guidance on how to manage. The phone number for telephone triage is: 570-824-3521 or Toll Free: 1-877-928-2621- Press #6
Note: Telephone Triage Service should only be utilized during non-business hours and holidays. (not to be used between 8:00am and 4:30pm Monday through Friday).
