Geriatrics and Extended Care
Congress has given the VA authority to provide long-term care benefits for qualifying Veterans. These benefits ensure that Veterans who qualify can receive a range of services which include:
Nursing Home Care (Community Living Center/CLC)
Dementia Care (CLC locked unit)
Respite Care (CLC)
Hospice and Palliative Care (CLC)
Short Stay & Long Stay (CLC)

This 100 bed inpatient nursing home offers short and long term care services as well as the specialty services of hospice, palliative care, dementia, respite care. Contact: Hope Danishanko, CLC Social Worker, 570-824-3521 x27545.
HBPC provides primary healthcare delivered by an interdisciplinary team of VA Staff in homebound Veterans. Services may include medical and nursing care, patient education, rehabilitation, nutritional counseling, medication review, social work intervention, and spiritual counseling. Home Health Aid (HHA)/Respite
Home Health Aid services are purchased for eligible Veterans. This service is considered when Veterans need assistance with activities of daily living. This includes assistance with bathing, dressing and grooming. Respite Care can help lower the stress the Veterans and their family caregiver may feel when managing a Veteran's long term care needs at home. Home Hospice provides care and support to terminally ill Veterans wishing to remain in their home until end of life. Contact: Katie Pisano, Care in the Community Chief, 570-824-3521 x24563.
Palliative Care offers pain management, and other symptom management services to Veterans diagnosed with a chronic irreversible illness. PCCT offers medical, social, psychological and spiritual support to patients and family as needed.
